a. Start out with a file 6 x .75 inches. RGB color, 72 ppi. Name your background layer BG and fill it with black. (D then Alt + Backspace)

b. Make another new file 6 x 6 px, With a transperant background.

a.In the bottom left side of the screen you will see a percentage that indicates the zoom on the image you are working on. Change it from 100% to 1600%.

c. Use the Single Row and Column Marquee Tools to make fill the left and top edge with black.

c. Go to Select>All, then go to Edit>Define Pattern...

- or you can download the pattern file from here and load it through the preset manager.

a. Now go back to your 6 x .75 in file.

b. Make a new layer by hitting the new layer button

c. Double click on the layer in the layers pallete and name it Grid. Now select the Paintbucket tool and change the fill mode to pattern. Then click on the Pattern dropdown box and select the grid pattern you just made. Now fill the new layer with your pattern. You may not be able to see it because the pattern will fill in black, but it's there.

Next hold Ctrl and click on the Grid layer to load its selection.

d. Choose this color from the color picker:

CFFFD0

e. Hit Alt + Backspace to fill the grid with the green you picked out.

a. Go to Filter>Blur>Gaussian Blur...
use these settings:

Radius: .4 pixels

b. Then hit Ctrl + F to repeat the filter.

a. Choose this color from the color picker:

b. Under the Grid Layer make a text layer.

a. Make a new layer, name it Scan.

b. Use the Rectangular Marquee tool (M) and make a selection like this:

c. Select the Gradient Tool (G), and configure the top bar like this.

a. Now using the gradient tool (G) make a gradient starting at the "X" and ending at the tip of the arrow.
